Made by using 5 Chaotic spikes on a Dragon claw.
This claw degrades over 10 hours of combat and once at 0%, it must be recharged by paying the Rewards trader at Daemonheim either 2,000,000 coins, or 200,000 coins and 20,000 Dungeoneering tokens.
If you no longer want this claw, then you have the option to destroy it to reclaim 50,000 tokens and the Dragon claw. It has an off-hand counterpart called the Off-hand chaotic claw.
You can augment this item by using an Augmentor to create Augmented Chaotic claw.
