Start by talking to Wizard Mizgog on the second floor of the Wizards' Tower, South of Draynor Village. He will tell you that a bunch of Imps have stolen his beads. Offer to help him.

"He will give you a note that states you must visit the 4 elemental altars around Gielinor: Air, Water, Earth, and Fire.
There is no order in which you must obtain the beads, and you do not have to enter any of the altars.

The Water Altar is located within the Lumbridge Swamp, just North-East of the South-Western most mining site. To the North of the altar should be the Phlegmatic imp.
Talk to him and you will need to get him a Bowl of hot water. The easiest way to get this is to take a bowl from the Lumbridge Castle kitchen, and fill it with water at the sink.
You can heat your Bowl of Water at the nearby range.
After you give him the Bowl of hot water, the Healthy imp will vomit out the Phlegmatic bead. Pick it up.
Note: The easiest way to get to the Water altar is to use the Wicked hood teleport and then exit the altar.

The Fire Altar is located North-East of Al Kharid, just North-West of the Duel Arena. To the South-West of the altar should be the Choleric imp. Talk to him and he will initiate combat, but he is easy to beat.
After he is defeated, the now Calm imp will vomit the Choleric bead at your feet. Pick it up.
Note: The easiest way to get to the Fire altar is to use the Wicked hood teleport and then exit the altar, or Ring of dueling teleport to Duel Arena, then run North-West.

The Earth Altar is located North-East of Varrock, just South of the Sawmill/Lumber Yard. To the North-West of the altar should be the Melancholic Imp. Talk to him.

Take your time. No one is rushing you to feel better.
Then, the now Happy imp will vomit the Melancholic bead at your feet. Pick it up.
Note: The easiest way to get to the Earth Altar is to use the Wicked hood teleport and then exit the altar.

The Air Altar is located in West Varrock, just East of Barbarian Village (Gunnarsgrunn). To the North of the altar should be the Sanguine Imp. Talk to him and you will challenge him to a race around the Air altar.

To win, you must hit all 16 of the white circles (checkpoints) before he does. He will be extremely fast at first, but once he gets to around 9 checkpoints, he will slow down. Below is an example of one of the checkpoints.

After you win, the now Relaxed imp will vomit out the Sanguine bead. Pick it up.

Once you have collected the beads, head back to Wizard Mizgog to claim your rewards.
