You make this item by using Rat's paper with An empty folder. This will insert the papers in the folder and if you have inserted 4 papers or less, then you get this item. When you insert all 5 papers into the folder, then you will get a Full folder instead. If you read it, it says: "The pieces of paper appear to contain lots of facts and figures. They look like accounts and lists of items. You're not sure what they all mean." If you destroy or lose this item, then you will need to retrieve the original empty folder and the papers separately.
