In the Meiyerditch Dungeon, the plushie is found on the coffin, in the northern room(blood altar inside).

Upon obtaining the plushie, you may squeeze it for a Shiny columbarium key, or turn it and the other 12 plushies, for a total of 13 plushies, in to Lex. Either way is possible to complete the miniquest.
